Create a Health Record

Create a Health Record

Maintain student health information including medical conditions, allergies, and emergency contacts.

Time required: 5-10 minutes

Prerequisites

• Health Admin or Nurse role
• Student record exists

Steps

Step 1: Navigate to Health Records

1. Go to OpenEduCat → Health → Health Records
2. Click Create

Step 2: Select Student

Step 3: Enter Basic Health Information

Blood Group	Blood type	”O+“
Height	Student height	170 cm

Vision	Visual acuity	”Normal”

Step 4: Document Medical Conditions

Allergies	Food, medication allergies
Chronic Conditions	Asthma, diabetes, etc.
Disabilities	Physical/learning disabilities
Medications	Regular medications

Step 5: Add Emergency Information

Emergency Contact	Person to contact
Contact Phone	Emergency phone
Family Doctor	Personal physician
Hospital Preference	Preferred hospital

Step 6: Save Record

Click Save to create the health record.

Add Medical History

In the Health Lines tab:

1. Click Add a line
2. Record past medical events:

Troubleshooting

Cannot view health records

Health records are sensitive. Check:

• You have health admin permissions
• Student consent is recorded
