Add Medical History Entries

Add checkup details and medical history entries to student or faculty health records.

Time: 2–3 minutes per entry Module: OpenEduCat Health Enterprise Permissions: Health User or Health Manager


Prerequisites

  • Health record exists for the student or faculty member
  • Health record is in Draft state (entries can only be added before confirmation)
  • Access to OpenEduCat → Health menu

Steps

1. Open the Health Record

  1. Navigate to OpenEduCat → Health
  2. Locate and click the health record to open the form view
  3. Verify the record shows Draft status in the status bar

2. Add a Medical History Entry

  1. Scroll to the Checkup Details section at the bottom of the form
  2. Click Add a line in the health line table
  3. Complete the entry fields:

Checkup DetailEnter the medical history description (required)
RecommendationAdd doctor’s recommendations or follow-up notes
  1. Press Enter or click outside the row to save the line

3. Add Additional Entries (Optional)

Repeat step 2 to add multiple medical history entries. Each entry creates a separate record linked to the health profile.

4. Save the Health Record

Click Save to persist all health line entries.

Common Use Cases

Recording Vaccinations

Use the Checkup Detail field to track vaccination history:

  • “Hepatitis B vaccination - Dose 1”
  • “COVID-19 vaccination (Pfizer) - Primary series”
  • “Influenza vaccine 2024-2025 season”

Tracking Regular Checkups

Document periodic health assessments:

  • “Quarterly health screening - all parameters normal”
  • “Monthly weight check - 52kg”
  • “Blood pressure monitoring - 120/80”

Medical Incident Documentation

Record health incidents and treatments:

  • “Mild fever - treated with paracetamol”
  • “Sports injury - right ankle sprain”
  • “Allergic reaction to peanuts - treated with antihistamine”

Tips

  • Date tracking: Always record accurate dates for medical history timeline
  • Detail specificity: Include specific symptoms, measurements, or observations
  • Recommendation follow-up: Use recommendations to track required actions
  • Chronological order: Entries display by date for easy history review

Portal Visibility

Medical history entries (health lines) are not visible to parents through the student portal for privacy protection. Only basic health measurements (height, weight, eye status) from confirmed records are displayed.



Security Notes

  • Health User: Can view and edit health lines on accessible records
  • Health Manager: Full access including delete capability
  • Multi-company rules restrict access to company-specific records
