Create Survey Templates

Design feedback and survey templates for course evaluations, faculty reviews, and general feedback collection.

Time required: 15-20 minutes per template

Module: openeducat_survey_enterprise, survey

User role: System Administrator, Academic Admin

Prerequisites

  • System Administrator or Academic Admin role
  • Survey module installed
  • Understanding of survey requirements

Steps

Step 1: Navigate to Survey Templates

  1. Go to Surveys -> Surveys
  2. Or OpenEduCat -> Feedback -> Templates
  3. Click Create

Step 2: Enter Survey Information

Step 3: Configure Survey Settings

Step 4: Create Question Pages

Organize questions into sections:

  1. Click Add a section
  2. Name the section (e.g., “Course Content”)
  3. Add questions within section

Step 5: Add Questions

For each question:

Step 7: Configure Scoring (Optional)

For graded surveys:

  1. Enable Scoring option
  2. Assign points to answers
  3. Set passing score

Step 8: Design Layout

Step 9: Save Template

Click Save to create the survey template.

Troubleshooting

Survey not accessible

Check survey state is In Progress and access settings are correct.

Responses not saving

Verify required questions are answered and form validation passes.

Cannot see results

Ensure user has Survey User or Manager access rights.
